It does not explain why others did not use it, whether access was easy, whether the service matched employee needs, or whether the people who used it improved.

Start with the definition of utilization

There is no single EAP utilization rate that can be interpreted without knowing how the program defines use, which employees are eligible, and what services are included.

The calculation can change depending on whether the provider counts registration, an initial contact, a counseling session, digital content, coaching, repeat visits, or dependent use. The denominator matters too. A rate based on all eligible employees is not the same as one based on enrolled members.

Peer-reviewed EAP research also shows why utilization should be treated as a measure of contact rather than a complete measure of effectiveness. An enhanced EAP intervention study reported a 53% increase in total EAP cases after changes designed to improve outreach and access, showing that program design can influence how much a service is used.

Before comparing utilization rates, ask exactly what sits in the numerator and denominator.

Low use can reflect access, privacy concerns, or fit

When utilization is low, the next question should be why.

Employees may not know the service exists, may not know how to access it, or may worry about privacy. Some may face long waits, limited provider choice, language or scheduling barriers, or a program that does not match the kind of support they are looking for.

Low utilization can reflect many different things, including limited awareness, access barriers, privacy concerns, scheduling difficulty, or a mismatch between the service offered and the support employees are looking for.

El enhanced EAP intervention study is useful because utilization increased after changes to outreach and access. It does not establish why utilization is low in every organization, but it shows that use can change when the way a service is offered changes.

Utilization and effectiveness are different measures

An employee using an EAP confirms that contact occurred. It does not show what happened after that contact.

Employers still need to know whether symptoms improved, work functioning changed, absence changed, appropriate care was reached, and whether the service helped the groups it was intended to support. They also need some view of the wider workforce, including people who never used the benefit.

Research on EAP outcomes shows why those questions need their own measures.

A prospective quasi-experimental study matched 156 EAP users with 188 non-users. At follow-up, the EAP group had greater reductions in absenteeism and presenteeism, while workplace distress did not show a significant difference.

A related study using the same participant counts found reductions in depression and anxiety symptoms among EAP users, and those changes were associated with reductions in absenteeism and presenteeism.

A 2024 quasi-experimental study found improved health-related productivity after EAP counseling, while absenteeism did not change at six months.

The findings differ by outcome, which is exactly why utilization should not be used as a stand-in for effectiveness.

What should an employer measure?

A useful EAP review separates four questions. This broader approach can also help benefits teams evaluate workforce needs beyond utilization.

Reach and access: Do eligible employees know the benefit exists, understand how to use it, and get appropriate support without unnecessary friction?

Who is being missed: Are there meaningful differences by location, shift, role, language, or other relevant employee group?

Outcomes among users: What changed after people received support? Depending on the program, this may include validated clinical measures, wellbeing, work functioning, satisfaction, continuation of care, absence, or other agreed outcomes.

Workforce-level context: What is happening across the broader employee population, including people who never used the EAP? Absence, leave, turnover, healthcare use, employee feedback, and repeated wellbeing measures can help answer that question.

Cost also belongs in the review, but financial claims need the same discipline. Vendor-reported ROI should be treated as vendor evidence unless the method and underlying data have been independently evaluated.

Questions to take into an EAP renewal

Before deciding that an EAP is underused, ask:

What exactly counts as utilization?

Which outcomes are measured with validated methods?

What population is used as the denominator?

Are enrollment and actual service use reported separately?

Which employee groups are using the service, and which are not?

What access barriers show up in wait times, provider availability, scheduling, language, or location?

What changes after someone uses the EAP?

What is changing across the wider eligible workforce?

Which provider-reported results have independent support?

A low utilization rate is useful information, but it is only one part of the evaluation.
